Heart of Abundance, LLC is happy to bill your insurance company for services rendered. We additionally support those who would like to move forward paying out of pocket. Please continue reading to understand insurance companies we are paneled with and our self-pay rates.

*All Credit Cards must have cvv code to be accepted. For this reason, many clients are unable to use CareCredit Healthcare Credit Cards for services.

We encourage all clients wishing to utilize insurance benefits to check in with their insurance company as each plan is different. Heart of Abundance, LLC Tax-ID is 83-2442786. General CPT codes that we will use are 90791 and 90837.

Some questions to ask your insurance provider:

  • Do I have mental health/substance abuse benefits?

  • Is this provider considered In-Network (provide NPI and Tax ID)?

  • Do I have a deductible? If so, how much is remaining? Does my deductible apply to these services?

  • Do I have a co-pay or co-insurance?

  • Is telehealth covered at plan benefits?

Self-Pay Rates

  • Initial Assessment/Evaluation 

    • 50 minutes $175.00

  • Individual Therapy 

    • 45- 50 minutes (regular session) $150.00

    • 100 minutes $210.00 (self-pay clients only, as most insurance do not approve this extended time frame)

  • Marital/Couples/Family Therapy

    • 45 - 50 minutes $175.00

    • 100 minutes $210.00 (self-pay clients only, as most insurance do not approve this extended time frame)
